Intravenous recombinant tissue plasminogen activator (IV rt-PA) is the sole proven medical treatment of acute ischemic stroke in the first 4,5 hours. But the most feared complication of IV rt-PA in acute ischemic stroke is symptomatic intracerebral hemorrhage. The incidence of unruptured intracranial aneurysms in the general population is 2% -3%. Most of them are asymptomatic. Here, we aimed to share a patient of acute ischemic stroke and coincidental intracranial aneurysm who was treated with IV rt-PA and did not develop any complications.
