AMAÇ: Diyabet inme için önemli risk faktörlerinden biridir. Diyabetin inmeli hastalarda uzun dönem takiplerinde mortalite üzerinde olumsuz etkileri olduğu bulunmuştur. Kısa dönem içindeki mortalite ve morbidite üzerine etkileri tartışmalıdır. Bu çalışmada diyabetik ve non-diyabetik inme hastalarında eşlik eden risk faktörleri ve kısa dönem içindeki prognoz arasındaki ilişkiyi incelemeyi amaçladık.
YÖNTEMLER: Çalışmaya ilk 24 saat içinde acil polikliniğe başvuran, ilk defa akut inme tanısı alarak yatırılmış hastalar alındı. Olgular diyabetik ve non-diyabetik hastalar olmak üzere iki gruba ayrıldı. Hastaların demografik özellikleri, eşlik eden hastalıklar, risk faktörleri ve laboratuvar tetkikleri incelendi. Hastalara 1. ayda Barthel indeksi, başvuru ve taburculukta National Institute of Health (NIH) inme skalası uygulandı.
BULGULAR: Diyabetik ve non-diyabetik grupta başvuru-taburculuk NIH, 1. aydaki Barthel indeksine göre tam bağımsızlık oranları arasında anlamlı farklılık saptanmadı. Barthel indeks puanlarına göre gruplandırıldığında 1-61 aralığında olan grubun (tam ve ileri derecede bağımlı olgular) başvuru kan glukozu değeri, Barthel indeksi 62-100 arasında olan gruptan (orta, hafif dercede bağımlı, tam bağımsız olgular) anlamlı olarak yüksek gözlendi, açlık kan glukozu arasında anlamlı farklılık gözlenmedi
SONUÇ: Çalışmamızda, diyabetik ve non-diyabetik hastalarda barthel indeksi puanları arasında belirgin bir fark olmamasına karşın, başvuru kan glukozu yüksek olan hastalarda morbiditenin daha belirgin olduğu sonucuna varılmıştır.
OBJECTIVE: Diabetes is one of the important risk factors of stroke.Diabetes has been found to have adverse effects on long-term follow up of stroke patients.Short term effects of diabetes on mortality and morbidity of stroke is controversial. In this study we aimed to investigate the relationship between diabetic and non-diabetic stroke patients concominant risk factors and short term prognosis.
METHODS: Patients who are registrated into Emergency Room in their fisrt 24 hour and hospitalised with a diagnosis of acute stroke were included into the study.Patients were splitted into two groups as diabetics and non-diabetics.Demographical characteristics,coexisting diseases,risk factors and laboratory findings of patients were investigated. Barthel index in the first month, National Institude of Health(NIH) stroke scale at admission and discharge were applied on patients.
RESULTS: There was no statistically significant differences between diabetic and non diabetic groups according to admission-discharge NIH and full independency ratios at first month Barthel index. As we grouped patients according to Barthel index points, admission blood glucose values of group of patients who are between 1-61(fully dependent and high degree dependent cases) are significantly higher than the group of patients whose Barthel indexes are between 62-100(low to moderete degree dependent and full independent cases).Fasting blood glucose levels didn’t differ signicantly between these groups.
CONCLUSION: In our study, there were no significant differences between the scores of Barthel index of diabetic and non-diabetic patients. But it was concluded that the patients which has higher reference blood glucose levels were more pronounced with morbidity.
