Derin serebral venlerin de tutulduğu atipik serebral venöz tromboz olgusu

Serebral ven trombozu, baş ağrısı, fokal defisit ve komayla karakterize olan, genellikle gençlerde ve çocuklarda görülen bir serebrovasküler olaydır. Biz 22 haftalık gebe derin ven trombozu olan hastanın görüntülemede tek taraflı izole talamus tutulumu saptadık, ancak tekrarlanan MRI’da her iki talamusunda etkilendiği saptandı. Bu olguda serebral venöz tromboz açısından risk faktörü olan hastada atipik de başlasa mutlaka MR venografi ile serebral venöz tromboz ekarte edilmesi gerektiği vurgulandı. Litaratürde derin ven trombozlarında kliniğin kötü prognozlu olduğu vurgulansa da vakamızda klinik
unfraksiyone heparin tedavisinden sonra oldukça iyi seyretmiştir.

Atypical cerebral venous thrombosis case with diffuze cerebral vein involvement

Patients with cerebral venous thrombosis may present with headache, focal neurological deficit and coma. Cerebral venous thrombosis is common in young adults. We presented a 22 years old pregnant women with deep vein thrombosis in whom unilateral thalamic lesion was found followed by bilateral thalamic involvement. In the presence of risk factors for cerebral venous thrombosis, atypical presentation should not exclude possibility of the diagnosis hence MR venography may be required. Although the prognosis of cerebral deep venous thrombosis is poor in litarature the prognosis of our case was excellent with heparin therapy.
