Akut dönem sonrası göz açma yanıtı geri dönen ama kooperasyon sağlanamayan olgularda (persistan) vejetatif durum (PVD), minimal bilinçlilik durumu (MBD) veya mezensefalik "locked-in" sendromu (LİS) gibi farklı tanıların sadece klinik bulgular temelinde konulması son derece zordur. Bu tanımlamalar prognoz ve tedavi seçimi konusunda son derece kritiktir. Sadece stimulus verilerek elde edilen pasif veya aktivite gerçekleştirerek yapılan aktif fonksiyonel manyetik görüntüleme (fMRG) ile istirahat fMRG bu amaçla kullanılabilir. Bu makalede klinik bulguları ile PVD, LİS ve MBD ayrımı yapılamayan üç olguya fMRG desteği ile pontomezensefalik infarkta bağlı LİS, pontin hemorajiye bağlı PVD ve global serebral iskemiye bağlı MBD tanıları konulmuş; bu tekniğin nöroyoğun bakım ve nörovasküler hastalıklar pratiğindeki önemi tartışılmıştır.

These descriptions are of uttermost critical importance in terms of prognosis declaration and treatment level selection. Stimulus-evoked and task-evoked functional magnetic resonance imaging (fMRI) or resting state fMRI can be used for this purpose. Three cases, in whom convincing discrimination of PVS, LIS and MCS was impossible to be clinically attained are herein presented and discussed. For the sake of fMRI technology, diagnoses of LIS connected to pontomesencephalic infarction, PVD due to pontine hemorrhage and MBD from global cerebral ischemia could be insured. Utility of fMRI in the neurocritical care and neurovascular disease practices is briefly presented
