İntraserebral hematom genişlemesini etkileyen olası faktörler

GİRİŞ ve AMAÇ: Spontan intraserebral kanamalı hastalarda hematom genişlemesi nörolojik kötüleşmenin önemli bir nedenidir. Hematom genişlemesini etkileyen faktörlerin belirlenmesi yüksek riskli hastaların daha yakından takibini sağlayacaktır. Bu çalışmada genişleme gösteren spontan intraserebral hematomlu hastaların klinik, laboratuvar ve radyolojik özellikleri incelenmiştir.
YÖNTEM ve GEREÇLER: Bu çalışma Eylül 1999 ve Haziran 2001 tarihleri arasında prospektif olarak planlanmıştır. Hastalar hematom genişlemesi olup olmamasına göre gruplandırıldı. Hematom boyutları AXBXC/2 formülüne göre hesaplandı. İlk BT çekimindeki hacim V1 kontrol BT hacmi V2 olmak üzere iki BT arasındaki hacim oranı (V2/V1) 1.5’den büyükse veya iki hacim arasında 10cm3’den fazla fark varsa hematom genişlemesi olarak kabul edildi.
BULGULAR: 21 hastanın beşinde hematom genişlemesi tespit edildi. Hematom genişlemesi olan hasta grubunta yüksek üre, kreatinin, kan şekeri, düşük fibrinojen seviyesi, bilinç bozukluğu ve lökositoz varlığı istatistiksel olarak anlamlı bulundu.

Probable factors affecting the development of the enlargement of the intracerebral hemorrhage

INTRODUCTION: The enlargement of hematoma in patients with spontaneous intracerebral hemorrhage is an important cause of neurological deterioration. The determination of the factors affecting the enlargement of hematoma will provide following up of the patients with high risk more closely. In this study, we investigated the clinical, laboratory, and radiological characteristics of hematoma enlargement in patients with spontaneous intracerebral hemorrhage.
METHODS: This study was planned prospectively between September 2001 and June 2001. The patients with spontaneous intracerebral hemorrhage were grouped according to the presence or absence of enlargement in hemorrhage. Volume of hemorrhage was calculated by using the formula of AXBXC/2. The volume in the first CT was determined as (V1) and in control CT as (V2). If the ratio between two of them V2/V1 was greater than 1.5 or if the difference between two volumes was greater than 10cm3 it was accepted as hemorrhage enlargement.
RESULTS: Of 21 patients, hemorrhage enlargement was detected in five patients. In the group of patients with hemorrhage enlargement, high level of urea, creatinine, blood glucose, systolic blood pressure; low level of fibrinogen, presence of defect in consciousness, and leucocytosis were found different statistically.
