Global aphasia without hemiparesis (GAWH) is an uncommon stroke syndrome involving receptive and expressive language impairment, without the hemiparesis typically manifested by patients with global aphasia after large left perisylvian lesions or of two separate lesions in the anterior and posterior left hemisphere language areas. Usually acceptable GAWH has been good recovery of verbal communicative abilities. We present two cases of GAWH with conflicting conclusions regarding pathogenesis, lesion localization, and recovery herewith literature.
